	Long Term Care Coordination for elders living alone includes: -finding needed services -24 hour a day availability -regular contact with family -comfort and support		Professional screening of nurses' aides and companions for partial day to 24 hour care. Assistance in negotiating salary and hours.
	A plan of care that may include: physicians, therapists, trust officers, attorneys and other support services.		Quality, affordable case management for residents of extended care homes.
