1-800-227-4165 Unum's toll-free help line is available for answers to your questions about your coverage or how to enroll. Newly Hired Employees Once eligible for this plan, you will have 30 days to sign up for Guarantee Issue coverage. (Guarantee Issue means that you do not have to fill out a medical questionnaire.) If you choose to enroll after the Guarantee Issue enrollment period, or choose benefits over the Guarantee Issue limits, you will be required to fill out a medical questionnaire. Effective Date of Coverage The effective date for those applicants passing medical underwriting between the 1st and the 15th of the month is the first of the month following their date of approval. For those approved between the 16th and the end of the month, their effective date is the fist of the second month following their date of approval.
