How many different family members can I submit?
Your employer’s Long Term Care Insurance Plan allows employees, spouses, siblings, parents (in-law), grandparents (in-law), and adult children to participate at discounted group rates, up to 40% below market rates. There is no limit to the number of family members you can submit to receive this information.

My close relatives have several Email addresses, which one should I enter?
You can enter whichever Email address you prefer.

How can I or my close relatives obtain a LTC Family Enrollment Kit by U.S. mail?
A LTC Family Enrollment Kit can be received via U.S. mail by calling UNUM at, 1-888-868-6745. You will need to identify your employer name and employer policy number. If you would like to receive a LTC Family Enrollment Kit by Email, please click here.
